Within MODC, we have an After Stroke Community Program which is a personalized recovery program that helps survivors, and their families move forward after a stroke. Our Mission is to deliver person-to-person support, educational opportunities, and innovative community-based programs that provide a connection to humanity, hope, and life after stroke. Our vision is for a world where stroke survivors and their families can recover and live active, healthy and connected lives.
